Year 6 to 7 Transition Documents

Growing out of the Priority 2 group’s transition project (led by Paul Chambers, Stoke-on-Trent City Council), the standardised transition forms have been developed to support Year 7 teachers in identifying curriculum deficits and gaps in pupils’ learning, so that attention can be targeted more effectively when pupils begin secondary school.

There is keen interest in this model from other parts of the country, and from the Department for Education, as an example of best practice.
